I am reading COUPLES THRIVING with ADHD and in Chapter 2 there is a symptom tracker listed. 

 

Does anyone have the link?

At the top of the page, click on the tab  Treatment & tools.  

From the Drop Down Menu, click on Online treatment Guide.  

Down the middle of the page, in the second section -

Thank you so much. Case of the juice in the fridge infront of me. Just couldn't see it.
